The Ayushman Bharat Digital Mission (ABDM), implemented by the National Health Authority [3], anchors India's digital health ecosystem through ABHA, a 14-digit consent-based health ID [2]. With 93 crore ABHA accounts and 104 crore linked records [1], its scale is unmatched — yet reaching the last citizen remains uneven.

Enabling foundations laid

• Federated DPI architecture: ABHA plus provider/facility registries and a consent manager enable interoperable, portable records [1].
• Inclusion-sensitive design: assisted and offline ABHA creation for low-connectivity areas [1].
• Efficiency at delivery point: "Scan and Share" cut OPD waiting from ~1 hour to 2–5 minutes [1].
• Incentives to onboard hospitals, labs and digital providers [1].

Challenges to last-mile inclusion

• Digital divide: patchy rural connectivity, limited smartphone/device access, and low digital and health literacy among the elderly, poor and tribal populations exclude the very groups UHC targets.
• Quantity over quality: high ID-generation numbers mask thin active linkage, duplication and dormant accounts.
• Privacy and trust deficit: sensitive health data under a consent framework [2] raises data-protection and cybersecurity concerns that can deter adoption.
• Supply-side gaps: voluntary provider onboarding, weak private-sector interoperability, and shortages of digitised facilities and trained staff in rural India.
• Language and usability barriers in citizen apps.

ABDM has convincingly built the rails; the frontier is now equitable last-mile access. Prioritising digital literacy, robust offline modes, DPDP-aligned data safeguards and rural provider onboarding can convert reach into genuine inclusion, advancing Universal Health Coverage and SDG-3.
