Evaluate the role of ESIC in delivering universal health security to India's workforce.

In this answer
  1. Strengths in delivery
  2. Limitations

The Employees' State Insurance Corporation (ESIC), a statutory body under the ESI Act, 1948, has evolved from a contributory sickness-insurance fund into a frontline vehicle for universal worker health security, reinforced by the four Labour Codes enforced on 21 November 2025 [1][3].

Strengths in delivery

  • Expanded coverage: beneficiaries have doubled to roughly 15 crore, while overall social-security coverage rose from about 19% to 64% of the population, aligning with ILO minimum-standard ambitions [1].
  • Preventive turn: the nationwide Annual Health Check-Up Initiative (7 May 2026) for workers aged 40+ shifts ESIC from curative dispensaries to systematic screening of NCDs like diabetes and hypertension [1][2].
  • Widened net: voluntary coverage for units under ten employees and mandatory coverage even for a single worker in hazardous occupations extend protection to the vulnerable [4].
  • Inclusion of gig/platform workers, first defined in the Code on Social Security, 2020, with Aadhaar-linked e-Shram IDs enabling portability [4].

Limitations

  • Geographical unevenness: ESIC hospitals and dispensaries remain concentrated in industrial belts, thinning access in newly notified rural districts [2].
  • Federal friction: labour being Concurrent, uneven state rule-alignment risks patchy rollout [3].
  • Delivery gaps: infrastructure and specialist shortages strain quality; gig-worker fund operationalisation is nascent [4].

ESIC is indispensable and increasingly universal in ambition, yet its promise hinges on plugging infrastructural and federal gaps. Strengthening ESIC as a preventive, portable, pan-India institution — anchored in DPSP Articles 41–43 — can make health security a genuine right for every worker.
