  1. Strengths: Pandemic response
  2. Strengths: Region-specific challenges
  3. Limitations

The Indian Council of Medical Research (ICMR) began in 1911 as the Indian Research Fund Association [3]. It is India's apex biomedical research body under the Department of Health Research. ICMR has proved strong in crisis research and in research on specific regions and communities. Its main weakness is turning that research into care at the last mile.

Strengths: Pandemic response

  • ICMR played a central role in developing the indigenous COVID-19 vaccine, which was used in India and abroad [1].
  • It hosts a COVID vaccine and clinical registry portal, which built national research infrastructure [3].

Strengths: Region-specific challenges

  • Tribal health: ICMR runs 10 Tribal Health Research Units and a Tribal Health Research Forum (2010) that links 16 institutes under NIRTH Jabalpur [4].
  • Sickle cell: It has validated 30 point-of-care tests used for screening in tribal-dominated states [5].
  • High altitude: The new Keylong (HP) Centre for High Altitude Medicine will study hypoxia, extreme cold, climate change and tribal lifestyles. It is a joint effort with the Armed Forces Medical Services, with AIIMS Bilaspur as the clinical partner [1].
  • The Keylong centre upgrades an existing field station. This gives year-round access to high-altitude populations for long-term cohort studies [2].
  • Its community-partnered model treats local people as "partners in research" [1].

Limitations

  • Research–delivery gap: ICMR has run tribal research units for years. Even so, the 2018 Expert Committee on Tribal Health found deep gaps in primary care and health workers [6].
  • Federal constraint: Filling health vacancies is mainly the States' responsibility, with NHM hard-area allowances as support [7]. A protocol fails if the local PHC has no doctor.
  • Overlap: DRDO's DIPAS already prepares acclimatisation schedules for troops [8]. This risks duplicating the Keylong centre's soldier-focused work.
  • Transparency: The Keylong announcement disclosed no budget or completion timeline [1].

Verdict: ICMR has built India's capacity for crisis and region-specific research well. Its influence weakens where delivery depends on other institutions.

Overall, ICMR has moved from a colonial-era fund to a research institution with national reach. To build on this, ICMR and the States should plan delivery together from the start. DHR and DRDO should agree in writing on who does what. Each centre should publish clear targets. Then research would turn into better health, not just research papers, advancing SDG 3 and the right to health under Article 21.
