  1. Stigma as a barrier: why the claim holds
  2. Medical access: an equal, often larger, barrier
  3. Assessment

India's leprosy prevalence is 0.56 per 10,000, below the elimination threshold. Yet in 2025-26, new child cases were 91.6% above projection and the Grade-2 Disability (G2D) rate was 1.34 per million against a target of 1 [1]. Both are signs of late detection. Stigma and weak access both cause it, but each matters most at a different stage of care.

Stigma as a barrier: why the claim holds

  • Fear of deformity is the root of leprosy stigma [2]. Patients hide early skin patches and come forward only when disability has set in.
  • Discrimination built into law: Law Commission Report No. 256 (2015) found laws barring affected persons from public office, public transport and certain trades [3].
  • Contacts stay hidden: families who fear being shunned avoid screening. This weakens Single-Dose Rifampicin prophylaxis for contacts [4][5].
  • The National Strategic Plan 2023-27 makes "zero stigma and discrimination" a core pillar [5], and the Sparsh campaign targets community stigma [2]. The State itself treats stigma as a central problem.

Medical access: an equal, often larger, barrier

  • Health-system gaps: the Central Leprosy Division (MoHFW) lists weak staff capacity and motivation, poor laboratory and referral services, and weak control of contacts [4].
  • Geography: prevalence is highest in tribal, remote States such as Chhattisgarh (1.66), Jharkhand and Odisha. 114 districts are still above the target [1].
  • Migration: high G2D rates in Chandigarh and Delhi point to migrant workers dropping out of MDT [1][4].
  • Active search works: the Leprosy Case Detection Campaign found 27,428 hidden cases in 2024-25 [6]. Reaching people's homes partly gets around stigma.
  • Complacency: mistaking "elimination" for "eradication" reduced vigilance [4].

Fig: Stigma and weak access feed one cycle

Stigma decides whether patients come forward and disclose contacts. Access decides whether the system finds and treats them. The statement is broadly valid, but it understates access. Where outreach is strong, stigma is partly bypassed. Where outreach is weak, removing stigma alone will not produce a diagnosis.

Stigma and access are two sides of the same failure. The way forward has three parts:

  • enact the anti-discrimination law recommended by the Law Commission;
  • build leprosy screening into RBSK, NUHM and Ayushman Bharat [4];
  • rank districts by child cases and G2D rate, not by prevalence.

Treating dignity under Article 21 as part of health care is essential to reach zero transmission by 2027 [5] and the SDG 3.3 target.
